Default Denied boarding to USA -- did have valid visa

Sounds fair enough to me, the passport with the visa in it was expired,
and therefore so was the visa.......

As long as the visa is not damaged and you have a valid passport of the same
nationality as the expired passport, the visa is valid.

Default Denied boarding to USA -- did have valid visa

According to the KLM website you can only check in on-line if you have
an e-ticket. What was not clear was whether you can do so with an
e-ticket for one of their flights purchased through a travel agent --
does anyone know?

Yes, no problem, but I think you do need the ticket number.
