Default MetroCard Transfers

So today I used my MetroCard to transfer from one bus to another, but
when I tried to transfer to a third bus I was charged full fare. Ok, I
guess the rule is that you're only allowed one transfer per fare.

Which leads me to ask ...

Are there any two bus stops in New York when you can't get from one to
the other with only one transfer?

Probably many, given that buses don't travel across some borough lines
many times of the day. And a trip between Staten Island and The Bronx
taking 2 or fewer buses and no trains? I'm guessing that's impossible,
except maybe for a few express bus options, which are expensive.
