Default Rail crash a few years back...

The news reports at the time, which was all I could remember, were
blaming the speed, although the official report implies that although
it was speeding it didn't contribute much to the derailment. The gist
at the time seemed to be that the amount of injury was remarkably
small and people were being interviewed about their lucky escapes.

This is the trouble with trying to remember something after eleven
years.
